Kindergarten Registration Info

PARENTS/GUARDIANS:

PLEASE REGISTER ALL KINDERGARTEN-AGE CHILDREN AT THE SCHOOL WHERE THEY WOULD ATTEND AS SOON AS POSSIBLE. 

Children must be five (5) years old on or before August 31 of the upcoming school year to register. During registration, parents will complete health and registration forms. Birth certificate and immunization records must be presented at the time of registration.

Pennsylvania State Law requires every child entering school for the first time to have: 4 DT (Diphtheria, Tetanus) with the fourth dose after age 4; 3 Polio; 2 Measles after age 1, 1 Mumps after age 2, 1 Rubella after age 1; 3 doses of Hepatitis B; and Varicella (chickenpox) immunization or history of disease.

If your child’s immunizations are incomplete, please call your physician or the Crawford County State Health Center (814) 332-6947 to arrange an appointment.
